 Spectrum Generations OFFERS CARE MANAGEMENT

 

Spectrum Generations’s home care division, Bridges Home Care, is pleased to announce that Patricia R. Greenleaf is now providing Geriatric Care Management consultation and on-going services to older adults and their families in the central Maine area.  Greenleaf recently completed the Certificate Program in Case Management at the University of Southern Maine and is now an affiliate member of the National Association of Professional Geriatric Care Managers (GCM).

Care Managers have extensive knowledge about the costs, quality, and availability of community services.  By conducting an individualized care-planning assessment, identifying problems, and researching options – Care Managers can put together an affordable plan of care and services to assist older persons and their families with the challenges of aging.

Greenleaf has worked for Spectrum Generations, the Central Maine Area Agency on Aging, for 14 years.  She is a Licensed Practical Nurse with 30 years of experience in hospital, nursing home, home health and home care settings. 

Greenleaf joins more than 1,600 GCM members across the United States who are committed to maximizing the independence and autonomy of elders while striving to ensure that the highest quality and most cost-effective health and human services are used when and where appropriate.
